AND WISHES TO PLAY IN A LEAGUE GAME, TOURNAMENT GAME OR EXHIBITION GAME.
>ONE TRIAL PERMIT IS GOOD FOR TWO GAMES WITHIN 15 DAYS OR A TOURNAMENT.
>SOME LEAGUES ONLY ALLOW A MAXIMUM OF 3 TRIAL PERMITS PER PLAYER PER SEASON.
>FORM CAN BE FILLED OUT AT THE SSA DISTRICT OFFICE BY ANY REPRESENTITIVE OF THE TEAM OR '
CLUB WISHING TO USE THE PLAYER.
>IN ADDITION TO THE FORM, A TRIAL PLAYER NEEDS AN OSA PLAYER BOOK.

DISTRICT OFFICE. A PASSPORT SIZE PHOTO IS REQUIRED. (THE ORIGINAL BIRTH
CERTIFICATE OR CANADIAN PASSPORT MUST BE SHOWN FOR PROOF OF AGE.) ORIGINALS
MUST BE PRESENTED-NO EXCEPTIONS APPLY.
** IF THE PARENTS OF THE PLAYER(S) DOES NOT WANT TO GIVE YOU THE CHILDS BIRTH CERTIFICATE OR PASSPORT THEY CAN COME AND SHOW IT TO US AT OUR OFFICE. HOWEVER, WE MUST BE NOTIFIED IF THE PARENT IS COMING.

THE FORM AND BOOK WILL THEN BE STAMPED AND SIGNED BY THE OFFICE AND THE PLAYER WILL
